FULL AND FINAL RELEASE

IN CONSIDERATION of the payment of the fee/charge herein, representing monies agreed upon by the party noted herein, as {name}, hereinafter referred to as the trainee) who does now do hereby release and forever discharge, Toma Dragicevic, Matthew Todman, Matthew Stuart and John Mayo (hereinafter referred to as the Trainors) the organizers of this training, and/or any other associated party related thereto and/or therein, from any and all actions, causes of action, claims or demands howsoever arising which heretofore may have been or may hereafter be sustained by said trainee, in consequence of any possible legal proceedings commenced or to be commenced and arising from any and all claims associated with the above noted Training, against the aforesaid, including all of each trainee and trainors’ successors, assigns, executors, etc. in association to either party.

The Trainee, or their representative (if under the age of 18) has this Full and Final Release of their own free will and under no duress and/or coercion by any party.

 IN CONSIDERATION of the payment of the fee/charge herein, representing monies agreed upon by the party noted herein, as Toma Dragicevic, Matthew Todman, Matthew Stuart and John Mayo does now do hereby release and forever discharge including indemnification for, REVO Jiu Jitsu, and/or any other associated party related thereto and/or therein, from any and all actions, causes of action, claims or demands howsoever arising which heretofore may have been or may hereafter be sustained by said REVO Jiu Jitsu, in consequence of any possible legal proceedings commenced or to be commended and arising from any and all claims associated with the above noted Training, against the aforesaid, including all of each trainee and trainors’ successors, assigns, executors, etc. in association to either party. 

The Trainee, or their representative (if under the age of 18) has this Full and Final Release of their own free will and under no duress and/or coercion by any party.
